Who our care finder service can help
Our care finders can help eligible people to apply for support through My Aged Care. Some people may be able to apply to My Aged Care directly themselves, or have a friend or family member who can help to apply.
You may be eligible for support from our care finders if you:
- need some aged care
- are homeless, or at risk of becoming homeless
- it’s hard to fill in the forms yourself
- it’s hard to talk to government services
- there isn’t a family member, friend or representative who is able to help you with the application
- you’re considered an older person
In case you aren’t eligible for support from us, we can suggest other available services.
Clients’ Stories
The care finder service is supported by the Western, North-West Melbourne, South-East Melbourne, Eastern, Murray and Gippsland Primary Health Networks through the Australian Government’s Primary Health Networks Program.
